Client Rights at Trellis Counseling
At Trellis Counseling, every person we serve is treated with dignity, respect, and compassion. Your voice, your safety, and your wellbeing matter to us. The following rights are protected under Oregon law (OAR 309-018) and reflect our commitment to high-quality, person-centered care.
Your Rights
You have the right to:
- Be treated with respect and to receive services without discrimination based on race, color, national origin, ethnicity, age, disability, sex, gender identity, gender expression, sexual orientation, marital status, religion, or veteran status.
- Receive high-quality, culturally responsive care that supports your health, safety, and wellbeing.
- Participate actively in your treatment, including helping to create your treatment plan and having the plan explained in clear and understandable language.
- Ask questions and receive information about your diagnosis, treatment recommendations, and alternatives.
- Consent to or refuse services, unless restricted by law.
- Have your privacy and confidentiality protected, including your personal health information, except where disclosure is required by law or necessary to prevent harm.
- View or request a copy of your record, consistent with state and federal regulations.
- Request a change to your record if you believe information is incorrect or incomplete.
- File a grievance or complaint without fear of retaliation and receive a timely, fair review.
- Receive written information about your rights and responsibilities in a way that is accessible and understandable.
- Have a support person involved in your care when appropriate and when you choose.
- Receive services in the least restrictive, most supportive environment possible.
