Your Right to Receive a Good Faith Estimate of Expected Charges
Under federal law, you have the right to receive an estimate of the costs for your mental health services before you receive them. This is part of the No Surprises Act, which protects clients from unexpected or unclear billing.
At Trellis Counseling, we want your care to feel transparent and predictable. This notice explains your rights and how we provide Good Faith Estimates for anyone who asks, and for all clients who are uninsured or choosing not to use insurance.
What Is a Good Faith Estimate?
A Good Faith Estimate (GFE) outlines the expected cost of non-emergency services. It includes:

The type of service you may receive

The estimated cost per session

How often services may occur

Your total estimated cost over a chosen period (for example: 3 months, 6 months, or 12 months of care)
This estimate is not a bill.
It is simply a tool to help you plan for your care.
Who Can Request a Good Faith Estimate?
You may request an estimate at any time. You are entitled to one if:

You are not using insurance

You are uninsured

You request an estimate for any reason, even if you are using insurance

You want a written overview of expected therapy costs before scheduling

What Your Estimate Includes
Your Good Faith Estimate will list:

The clinician providing your care

The service code(s) typically used (e.g., intake, individual therapy)

The fee for each service

Your estimated number of sessions

The projected total cost for the time period you choose

Additional information if your care plan changes
Therapy is a highly individual process. Some people attend weekly for a period of time; others attend twice monthly or as needed. Because of this, your actual costs may vary depending on your needs and your goals.
If your treatment plan changes, you can request an updated estimate at any time.
What to Do If You Receive a Bill That Seems Higher Than Expected
If you receive a bill that is $400 or more above the amount listed in your Good Faith Estimate, you have the right to dispute the bill.
To dispute a bill, you can:

Start a dispute process with the U.S. Department of Health & Human Services
You must begin the dispute process within 120 days of receiving the bill.
